Summary:
The Hospice Nurse Practitioner collaborates routinely with inpatient facility staff and the Hospice Physician.
Qualification Requirements
- Registered Nurse currently licensed to practice nursing in the state of NC.
- Nationally Certified Nurse Practitioner (geriatric, adult, family) currently approved to practice in the state of NC
MSN preferred. - Minimum of 3 years experience as a nurse practitioner.
- Previous experience working with terminally ill patients preferred.
- Maintain current CPR.
Essential Functions

- Creates positive experiences for internal and external customers that will meet their expectations. Displays a high degree of courtesy, tact, and knowledge of services provided by the agency in all contact with staff, patients, and visitors.
- Provide attending services to patients admitted to the facility as needed. Services include but not limited to; (a)pain/symptom control (b) management of chronic conditions when warranted (c) collaborate with family members as needed (d) face to face visits and (e) assist with discharge plans when necessary.
- Provide attending services for patients in the home hospice program as needed to include; (a)Assessment/evaluation (b) face-to-face visits and (c) follow up visits as needed.
- Assists agency with achieving positive patient outcomes by: (a) providing skilled nurse practitioner care as outlined in the plan of treatment, according to 3HC's policies and procedures and as allowed by the Practice Act of the N. C. State Board of Nursing; (b) scheduling and performing face to face visits with hospice patients to evaluate appropriateness for services as outlined by conditions of participation, (c) communicates regularly with the IDG team regarding visit findings and suggestions, (d) meets routinely with Hospice Physician as collaborating physician as outlined by N.C. Board of Nursing. (e) ensures collaborative practice agreement is reviewed and updated as outlined by the board of nursing.
- Documents in the medical record electronically and completes documents and attestation statements as outlined by the Conditions of Participation.
- Maintains regular communication with supervisor and communicates problems on a timely basis following chain of command. Communicates information using current processes and technology available to 3HC.
- Serves as a liaison between the agency and the medical community and is a resource person in community education.
- Demonstrates an interest in personal and professional growth for self and staff: (a) attends and participates in workshops, seminars, webcasts to keep abreast of current changes in rules, regulations, relating to job functions and department; (b) does individual reading and research to meet or exceed CEU requirements; and (c) recommends educational opportunities for staff and encourages participation; (d) attends and participates in mandatory in-services 100% of the time; (e) attends and participates in non-mandatory in-services and other educational opportunities as offered and attends IDG meetings upon request.
- Demonstrates a willingness to be cost effective in the use of agency resources, the monitoring of waste, and the proper and safe use of supplies and equipment.
- Adheres to 3HC's Personnel Policy and performs other appropriate duties as assigned by supervisor to promote the successful operations and future growth of 3HC.
