At 365 Hospice, we believe in providing compassionate, patient-centered care to those we serve and their families. As a family-owned and operated hospice, we know that quality care starts with a supportive, dedicated team. We are currently seeking an On-Call Registered Nurse to cover weekend shifts and provide exceptional end-of-life care. This on-call position would be from 4:30pm on Friday until 8:00am the following monday morning. This would be a weekend position and be salaried at full time. The on-call position is only required to leave your house when called out
Key Responsibilities
- Provide skilled nursing assessments and care for hospice patients in their homes or facilities.
- Respond promptly to urgent calls and make home visits as needed.
- Collaborate with the hospice team to ensure patients and families receive comprehensive support.
- Educate patients, families, and caregivers on hospice philosophy and plan of care.
- Document all care provided in compliance with hospice standards and regulations.
- Completes an initial assessment of patient and family to determine hospice needs.
What We’re Looking For
- Current Registered Nurse (RN) license in Pennsylvania.
- Minimum 1 year of clinical nursing experience (hospice or home health preferred).
- Strong critical thinking, communication, and compassionate care skills.
- Ability to work independently while being part of a collaborative team.
- Reliable transportation and valid driver’s license.
