We’re seeking a Registered Nurse Case Manager who is passionate about making a difference. As a key part of our interdisciplinary team, you’ll oversee and coordinate the care of hospice patients, ensuring they receive the highest level of clinical and emotional support.
What You’ll Do
- Manage a caseload of hospice patients, providing comprehensive nursing care
- Develop and implement individualized care plans
- Communicate closely with patients, families, and the hospice team to ensure comfort and understanding
- Provide education and emotional support to families during end-of-life care
- Ensure documentation meets compliance and quality standards
What We’re Looking For
- Active RN license in Pennsylvania
- Hospice or home health experience preferred (but we’ll train the right nurse!)
- Strong communication and organizational skills
- Compassionate, patient-focused mindset
- Valid driver’s license and reliable transportation
