Care Coordinator – Support Services
Location: Van Nuys, CA.| Full-Time

You’ll oversee Direct Support Professionals, collaborate with families and Regional Centers, and ensure every client receives high-quality, compassionate care.

- Required Availability- Monday-Friday 8:00am-5:00pm evenings and weekends as needed.

Your Role:
- Supervise and coach DSPs to deliver exceptional services
- Ensure families receive consistent, high-quality care
- Collaborate with Regional Centers, clients, and behavioral agencies
- Conduct staff meetings, professional development, and utilization reviews
- Provide clinical guidance and mentorship
- Maintain accurate documentation and communication
What We’re Looking For:
- 1 year clinical experience working with individuals with diverse needs
- Supervisory experience (preferred)
- Strong leadership, organization, and communication skills
- Ability to work independently and collaborate with a multidisciplinary team
- Comfortable in diverse home environments and culturally responsive
- Bilingual Spanish Required
Education & Certifications:
- High School Diploma/GED required; pursuing bachelor’s degree preferred
- Must pass background check/Live Scan
- TB clearance, CPR/First Aid, and Crisis Prevention Certification
