What are the responsibilities and job description for the HCB Traditional/PDS Case Manager position at 2nd Home Adult Health Care?
Position Summary:
As a case manager, you will play a vital role in facilitating interactions among case managers, clients, caregivers, and physicians to ensure the daily fulfillment of clients' health and medical requirements. You will operate under the guidance and supervision of the case manager supervisor.
Qualifications:
(PER 907 KAR 7:005 and 907 KAR 7:010)
· Agency case management staff who come into direct contact with waiver participants must have been hired before November 11, 2023,
or meet the following qualifications:
· Bachelor’s degree in social work/Human Services or related field; OR
· Bachelor's degree in any field not closely related AND one year of human services related experience; OR
· *An associate degree in a behavioral science, social science, or a closely related field AND two years human services related experience*; OR
· Three years of human services related experience.

Relevant experience may include:
o Experience as a case manager or in a related human services field
o Certified Nursing Assistant experience

o Personal Care Assistant experience
o Paid professional experience with aging and/or disabled populations or programs as a Case Manager, a Rehabilitation Specialist or Health Specialist, and/or Social Services Coordinator
o Assessment and care planning experience with clients
o Experience in working directly with persons with intellectual, developmental, or other types of disabilities or mental illness.
o Work aiding individuals and groups with issue such as economically disadvantaged, employment, abuse and neglect, substance abuse, aging, disabilities, prevention, health, cultural competencies, inadequate house
OR
- Be a registered nurse (RN) currently licensed in Kentucky as defined in KRS 314.011(5) who has at least two (2) years of experience as a professional nurse in the field of aging or disabilities; or
- a master’s degree in a health or human services field from an accredited college or university.
- Completes Department-approved, waiver-specific training and is monitored for competency on topics including, but not limited to abuse, neglect, exploitation and incident reporting, medication administration, professional boundaries, trauma-informed care, and person-centered thinking.
- Completes Department-approved case management training.
- Has the ability to:
o Communicate effectively with a participant in the participant’s preferred manner of communication and with the participant’s family.
o Read, understand, and implement written and oral instructions.
o Perform required documentation.
o Facilitate the participant’s person-centered team; and
o Demonstrate competence and knowledge of topics required to safely support the participant as described in the participant’s PCSP.
- Undergoes pre-employment screenings.
- Is certified in CPR and First Aid
Responsibilities:
a. Effective Communication: Your foremost duty is to communicate in a manner that prioritizes the participant's best interests.
b. Needs Identification: You are expected to identify and address the specific needs of each participant effectively.
c. Language Competence: Proficiency in the participant's language is essential, either through personal knowledge or the use of interpreters.
d. Sensitivity and Awareness: Demonstrate heightened sensitivity to the unique perspectives through which participants interact with their surroundings.
e. Coordination of Services: Take charge of coordinating services by facilitating team meetings involving representatives from all agencies involved in implementing a participant's Person-Centered Service Plan (PCSP).
f. Inclusion of Stakeholders: Involve the participant, their guardian, or legal representative in the case management process to ensure their active participation.
g. Documentation: Thoroughly document all interactions and communications the participant has with other agencies involved in the PCSP implementation.
h. Regular Contact: Maintain regular contact with the participant, either through monthly communication or face-to-face visits, with a minimum of one in-person visit between the case manager and participant every other month, particularly in an adult day health center or the participant's residence.
i. Residence Meetings: Organize at least three meetings annually at the participant's residence to ensure that services align with the participant's PCSP. If discrepancies arise, be prepared to issue a corrective action plan.
j. Conflict-Free Case Management: Adhere to the principle of Conflict-Free Case Management to maintain impartiality.
Compliance:
Case Managers must abide by the standards identified in 2nd Home’s compliance plan, HIPAA plan all other policies and procedures.

The requirement involves the knowledge, skill and ability to perform all position duties. Language and Communication Skills:
Advanced interpersonal and communication skills to secure cooperation, interpret policies, exchange information, and provide professional judgements in continuous contact with management personnel, colleagues and other employees. The case manager works with a range of adult clients of various ages with multiple types of health care needs. Ability to listen attentively and speak clearly and distinctly in receiving and responding to inquiries.
Certificates, Licenses, Registrations:
Training and certification in CPR within the first three months of employment and recertification as required. Each employee must have a PPD test annually or be determined to be free of symptoms of tuberculosis if there was a previous positive PPD.
