Job Overview
2nd Family is hiring a Part-Time Registered Nurse (RN) Care Coordinator to support clients and families through high-quality care coordination. This role offers remote flexibility, manageable caseloads, and meaningful clinical impact, with occasional in-home assessments in Wauwatosa, Menomonee Falls, Brown Deer and West Allis.
Key Responsibilities
- Serve as the primary nursing contact for assigned clients and their families
- Coordinate and oversee individualized plans of care
- Conduct initial nursing assessments and required 90-day reassessments in clients’ homes
- Communicate with, supervise, and support CNA/GNA caregivers
- Provide guidance and problem-solving related to client care needs
- Address client and family concerns to ensure smooth care delivery
- Remain available during designated on-duty hours (7:00 a.m. – 7:00 p.m.)
- Attend one monthly company meeting
Schedule & Flexibility
This is a part-time role with a flexible schedule. Most work is remote and phone-based. Initial assessments typically occur within 24–48 hours, so some short-notice availability is required. Outside of on-duty hours, communication is limited to emergency situations only.
Compensation & Pay Structure
- $100 per case, per bi-weekly pay period for ongoing care coordination
- $50 per completed in-person assessment
- Initial caseloads typically start at 3–5 clients, with growth opportunities
Qualifications
- Active Registered Nurse (RN) license in Wisconsin
- 2 years of nursing experience preferred
- Strong communication, organization, and care coordination skills
- Ability to work independently in a remote environment
- Reliable transportation for in-home assessments
