The Patient Support Specialist (PSS) plays a key role in ensuring a positive experience for our patients and their families. This position focuses on building strong relationships with families, checking in on their experience with our services, supporting care coordination, and assisting with scheduling and follow-ups.
This role serves as a bridge between patients, families, clinicians, and physicians, helping ensure continuity of care, satisfaction, and retention of services in a home health and hospice environment.
Patient & Family Engagement
- Proactively reach out to patients and family members to build rapport and maintain ongoing communication
- Check in with families to ask how our services are going and address concerns or questions
- Support patient and family satisfaction and service retention
- Communicate with empathy, professionalism, and cultural sensitivity
Follow-Ups & Feedback
- Conduct follow-up calls after visits or services
- Guide families through patient satisfaction surveys and ensure completion take it out
- Document feedback and escalate concerns to the appropriate clinical or administrative team
- Help identify opportunities to improve patient experience
Scheduling & Care Coordination
- Assist with scheduling appointments with physicians, clinicians, and care team members
- Coordinate visits between patients, families, and doctors if required
- Confirm appointments and provide reminders to families
- Serve as a point of contact for non-clinical coordination needs
Administrative & EMR / CRM Support
- Update patient records and interaction notes in the EMR / CRM system
- Maintain accurate documentation of calls, follow-ups, and survey results
- Assist with basic medical office and care coordination tasks as needed
- Collaborate closely with clinical and operations teams
