To assure the timely and accurate administration of medications and IV solutions to patients pursuant to a therapeutically valid prescribed order and in accordance with all state and federal regulations as well as established peer standards, hospital policies, procedures and protocols. Working with the Pharmacy Manager to manage the daily operations of all areas of drug distribution; supervises Pharmacy Technicians and Pharmacy Externs/Interns; follows pharmacy policies, procedures and protocols; participates in Performance Improvement activities and maintains readiness for JCAHO accreditation. Responsibilities: Clinically responsible for evaluating therapeutic rationale of all medication orders, providing information/education as needed, and for producing accurate medication administration records. Accurately follows various Clinical Pharmacy Protocols; High Dose Amino glycosides, Weight Based Heparin, Parenteral Nutrition, Adverse Drug Reaction Report, Renal Dosing. Verifies all aspects of medication order & follows AMAR policy. Performance Improvement – Medication Process. In response to new information or specific requests, educates medical, nursing, and pharmacy staff in a timely and accurate manner. Familiar with pertinent pharmacy, nursing service, and house-wide policies and procedures to facilitate accurate medication administration and optimal therapeutic results. Distributive responsibility for accurate and timely administration of Medications and IV solutions. Monitors efficient receipt of physician’s orders, processing of orders, and delivery of medications. Checks all medications for hourly distribution rounds. Knowledge of Multitask software program and pump hardware, including proper concentrations, labels, and correct exiting. Knowledge of specific interactions affecting All in One TPN’s and completion of Performance Improvement monitor for Parenteral Nutrition. Knowledge of Pyxis MedStation and Pharmacy console: demonstrates ability to load/unload medications, troubleshoot common problems (drawer failure, controlled substance discrepancy, etc.), troubleshoot Pyxis/Meditech issues, and reviews “meds ordered not loaded” reports for prompt attention. Required Knowledge & Skills: Education: Bachelor’s Degree in Science/Pharmacy or Doctor of Pharmacy (Pharm D) from an Accredited School of Pharmacy. Experience: Hospital experience including Unit Dose and IV Additives. Certification/Licensure: Current Massachusetts Pharmacy Registration. Computer literate. Good interpersonal and communication skills (written & verbal) required. Knowledge of Pyxis and Meditech.
