Job Description

Title: Direct Care Worker

*Reports To: Home Manager/Supervisor

Job Summary:

The direct care position provides direct supervision to the mentally impaired and developmentally disabled individuals who reside in the home. Direct care supervision responsibilities also include: protecting and affirming the rights of individuals while assisting them to achieve greater independence. By creating an environment responsive to their needs, care givers are expected to be respectful of every individual and willing to assist in all areas of daily living. Direct care staff is hired to work and assist all individuals in the home and provide a safe and comfortable living environment.

Essential Duties and Staff Responsibilities: Each direct care worker must be able to implement all aspects of each resident’s individual treatment plan as well as independently operate and complete all household duties. Other duties may be assigned.

· Carry out all job responsibilities associated with direct care and home operations.

· Implement each resident’s Individual Plan of Service on a daily basis.

· Implement All policies, procedures, guidelines and/or legal commitments to residents as required responsible agencies or Department of Social Services and 2 Foot Prints Inc.

· Participate as a constructive team member in formulating and implementing matters pertaining to the operation of the home and resident programs.

· Be an advocate for all residents.

· Communicate clearly, verbally, in writing and documented the necessary forms for all incidents pertaining to home operations in a timely manner in the following areas:

o Resident/Visitor movement to or from the home.

o Unusual incidents involving clients, staff or other persons affecting the home.

o Seizures, accidents or other medical emergencies.

o Medication

o Noticeable changes in resident conduct, either positive or negative, in the daily log.

· Organize, supervise, instruct and interact with residents in their daily program activities, using the resident’s Individual Plan of Service as a basis.

· Transport residents to and from workshop/school and community activities and supervise residents when appropriate.

· Know and follow emergency home procedures for fire, tornadoes, accidents or other serious incidents affecting staff and resident welfare.

· Assist parents/guardians and volunteers in understanding and working toward implementing normalization principles when in contact with residents.

· Maintain the quality of the home environment by taking initiative in (when required and as appropriate):

o Household cleaning

o Minor repairs and maintenance of home property and grounds

o Nutritional Meal preparation and Disinfect all areas

o Doing laundry, report any maintains or malfunctioning equipment, or vehicle

· Substitute for other program staff as requested and appropriate.

· Participate in training programs as required and arranged by 2 Foot Prints Inc.

· Conform to ALL policies and procedures set forth by 2 Foot Prints Inc...

· All other duties as assigned.

· Keep vehicles and homes clean at all times

· Assist new consumer with their IPOS/PCPS

Shift Requirements - All Employees

*The following must be checked regularly*

- Medication Sheet is signed

- Pill count and signed form – weekly (Sunday)

- Medication refill sheet updated- medications placed on refill sheet * 7 days before medication runs out.

- ALL consumer appointments written documented on calendar with:

o Time

o Physician name

o Contact number

- *Each consumer should have a monthly appointment w/Psychiatrist for a medication review.

- ALL pre-placement consumers should have a follow-up appointment w/Psychiatrist for a medication review within 7 days of entering the home.

- Check status of client appointments in accordance with transportation.

- Progress notes completed – per shift (daily)

- Check all phone messages

- Verify hours worked are correct - time sheet is complete and signed

- ALL shifts are properly covered - NO Dumping Consumers!

- Verify staff’s compliance with their duties and responsibilities.

- Performance reviews - Annually

- Cleaning and responsibilities sheet are being completed and signed.

- Review staff documentation.

- Auditing the consumers book and documentation – make sure all books have the required documentation, form, reports, and IPOS.

- Fax bed occupancy report to provider weekly when applicable – (Monday)

- Knowledge of general operation of each home:

· Staff working

· Client/Consumer’s residing at home

· Any special needs of consumer

- Additional staff training and discipline

- Ensure all homes are in compliance with upcoming inspections or follow-up inspections.

Qualifications:

1. Be a minimum of 18 years of age.

2. Minimum of High School education, GED or equivalent.

3. Be in good physical condition and submit a report of Physical Examination and

a negative TB test as a precondition of employment.

4. A valid Michigandriver’s license/State Identification.

5. Complete all VCE online training required by State of Michigan and contracted entities and attend Recipient Rights training as well as additional trainings

6. Must be able to complete required reports and follow written and oral

instructions that are related to the care and supervision of the residents

7. Must be suitable to meet the physical, emotional, intellectual and social needs of each resident.

8. Must be capable of handling emergency situations.

9*. Competent in All *of the following areas, prior to performing assigned tasks:

A) Reporting Requirements

B) First Aid & CPR

C) Personal Care, Supervision and Protection

D) Resident Rights

E) Safety & Fire Prevention

F) Prevention and Containment of Communicable Diseases

10. Must have the physical and mental capacity so as to not negatively affect either

the health of the resident or the quality of care.

Pay: From $17.13 per hour

Work Location: In person
