OB SUMMARY:
The purpose of this position is to assist the activity director in implementing an activity in compliance
with requirements to meet residents’ needs.
RESPONSIBILITIES AND MAJOR DUTIES:
1. Implement programs of individual and group activities based on residents’ needs.
2. Complete the activity component of the resident assessment within 14 days of admission and
review and update the program as necessary but at least quarterly.
3. Use the resident assessment protocols (RAPs) to determine whether to proceed to care
planning for concerns related to a resident’s activity.
4. Gather data and document required ancillary assessments including but not limited to resident
assessment protocol summary documentation.
5. Identify and document in the resident’s care plan, the activity interests, needs and concerns of
the resident.
6. Document all interaction with resident and/or family in the assessment, care plan and progress
notes as required by federal and state requirements.
7. Participate as an active member of the interdisciplinary team and attend all resident care
conferences.
8. Assist in planning and implementing Reality Orientation programs when appropriate.
9. Plan and conduct activity programs to assist residents to return to the community.
10.Assist in planning and conducting Residents’ Council meetings.
11.Assist in planning and implementing evening and weekend functions as necessary.
12.Take part in in-service education programs and attend all in-service sessions relating to the
Activity Program.
13.Implement community events related to residents’ interests.
14.Schedule and implement room visits and in- room activities for residents unable to leave their
rooms.
15.Schedule and implement indoor and outdoor activity programs.
16.Maintain an activity attendance record for each resident.
17.Maintain an adequate supply of materials to implement activity programs.
18.Observe infection control procedures.
19.Follow Residents’ Rights policies at all times.
20.Adhere to all facility policies & procedures.
21.Ensure adherence to the Corporate Integrity Program and the Performance Expectations at all
times.
22.Perform other related duties as directed by the Administrator and/or person of authority
