The AERS program is a state-mandated initiative designed to help functionally and chronically disabled adults and children remain independent and avoid institutionalization. In this role, you'll be a crucial guide and advocate, shaping how clients interact with support systems.
Here's what you'll do:
- Conduct comprehensive assessments: Analyze medical and psychosocial information, conduct home visits, and evaluate client needs to identify appropriate services.
- Develop personalized care plans: Help clients access home and community-based services that empower them to maintain independence in the least restrictive environment.
- Coordinate and connect: Refer clients to resources, consult with professionals, and coordinate services to ensure a seamless support system.
- Champion client autonomy: Advocate for client rights, promote self-determination, and ensure fair access to vital services.
- Document for impact: Maintain accurate records to track progress, inform service decisions, and contribute to the ongoing improvement of the AERS program.

- Holds a valid State of Maryland RN license (ASN minimum).
- Has 2 years of experience in medical-surgical/geriatric and/or pediatric case management.
- Possesses keen assessment skills and a knack for tailoring support to individual needs.

- Communicates with clarity and compassion, building trust and rapport effortlessly.
- Is a tech-savvy whiz comfortable navigating electronic medical records.

- Must be able to pass a thorough background investigation.
- Has reliable transportation and a valid driver's license equivalent to a noncommercial, class C Maryland driver's license.
Candidates must provide:
- TB Screening (within past 12 months – skin test or Quantiferon; chest X-ray if positive)
- Influenza Vaccination Attestation or Proof (current season; exemption documentation if declined)
- CPR Certification (valid and current)
