What you’ll do
- Deliver comprehensive nursing care to hospice patients in various settings including homes, nursing facilities, and hospitals.
- Conduct thorough patient assessments to identify needs and develop individualized care plans.
- Administer medications and treatments as prescribed, ensuring adherence to safety protocols and best practices.
- Provide emotional support and education to patients and families regarding disease processes, treatment options, and end-of-life care.
- Collaborate with physicians, social workers, and other healthcare professionals to coordinate patient care effectively.
- Monitor vital signs and manage symptoms such as pain and discomfort using advanced techniques including IV infusion and acute pain management strategies.
- Maintain accurate medical documentation in compliance with HIPAA regulations and organizational standards.
- Participate in ongoing training and development to enhance skills in hospice & palliative medicine.
Basic qualifications
- Valid nursing license with experience in hospice or palliative care settings.
Preferred qualifications
- Proficiency with electronic medical records (EMR) systems such as Epic or Cerner.
- Experience with critical care transport or within a Level I or II trauma center.
- Knowledge of ICD-9/ICD-10 coding for accurate medical documentation.
- Familiarity with geriatric care principles, including memory care and working with individuals with developmental disabilities.
- Skills in performing procedures such as suturing, venipuncture, catheterization, and administering injections.
