Responsibilities

- Coordinate post-acute care services, including discharge planning and outpatient follow-up, ensuring continuity of care across settings.
- Monitor patient progress through regular visits and telehealth check-ins, adjusting care plans as needed based on diagnostic evaluations and clinical findings.
- Collaborate closely with physicians, therapists, social workers, and other healthcare professionals to optimize patient outcomes.
- Manage medical records accurately while maintaining compliance with HIPAA regulations to protect patient privacy.

Skills
- Extensive critical care experience with a strong background in hospital medicine, emergency medicine, or ICU settings.
- Experience working within Level I Trauma Centers or Level II Trauma Centers, demonstrating advanced trauma management skills.
- Proven expertise in infectious disease care, post-acute care coordination, and discharge planning processes.
- Knowledge of medical terminology, physiology, and diagnostic evaluation techniques essential for comprehensive patient assessment.
- Skilled in utilization management, case management strategies, and managed care principles to ensure efficient resource use.
- Familiarity with inpatient and outpatient clinical environments including pediatrics (PICU), geriatrics, behavioral health, and medical-surgical units.
- Strong communication skills for effective intake procedures, medical records documentation, and interdisciplinary collaboration.
