Hourly Pay Range: $21.08 - $31.62 - The hourly pay rate offered is determined by a candidate's expertise and years of experience, among other factors. Position Highlights: Position: Community Health Worker Location: Chicago, IL (with the ability to travel to other locations locally) Full Time Hours: Monday-Friday, 8:00a-4:30p M-F with evening and Saturday availability as needed. A Brief Overview: A frontline public health worker who is a trusted member of and/or has a deep connection or understanding of the community served. This trusting relationship enables the CHW to serve as a liaison/link/intermediary between health/social services and the community to facilitate access to services and improve the quality and cultural competence of service delivery. What you will do: Assess and address patient’s needs in the area of housing, food insecurity, transportation or other non-medical areas that influence a patient’s health outcomes (Social Determinants of Health). In partnership with patient care teams, provides support for patients in gaining access to medical services, screenings, prescription medication and durable medical equipment. Assist patients with getting access to social services and other community-based resources. Follows up with patients via phone calls, home visits and visits to other settings where patients can meet. Maintain accurate documentation of patient contacts and document patient’s needs, action plan and outcomes in the medical record system. Represents the organization at various community events. Participate in continuing education, trainings and CHW organizations. What you will need: Minimum Education Level High school diploma, GED or equivalent. Community Health Worker, Health Coach, Patient Advocate Certification and/or equivalent preferred Experience 2 Years Community-Based Experience. Strong passion, leadership and interpersonal skills necessary to effectively work with multi-cultural, diverse populations. Ability to effectively engage in conversations, motivate and encourage patients to take steps to maintain and improve their health. Creative thinker with ability to problem solve and brainstorm solutions. Ability to work collaboratively as a part of a patient care team. Ability to work independently with minimal supervision. Leadership and community education experience. Basic computer skills required. Experience with EPIC EMR preferred. Fluency in Russian, Spanish, Urdu, Hindi, or Arabic a plus.
