Summary Coordinates the services for solid organ transplant patients and communicates with the multidisciplinary patient care team, nephrologists, and dialysis centers, staff, and other healthcare providers. Job Duties Coordinates admission of potential donor and recipient to hospital for surgery. Coordinates care for transplant patients in the inpatient setting. Coordinates organ transplant with Organ Procurement Organization, transplant candidate, physicians, and hospital staff. Evaluates potential transplant candidates and coordinates pre-transplant evaluation for potential transplant recipients. Manages pre-transplant patient care, evaluation, and management in the outpatient setting. Participates and coordinates in post transplant care of patients. Participates in 24 hour on call availability for transplant program coordinators and physicians. Reviews medical history and suitability to donor; coordinates schedules of medical tests and reviews test results to ensure optimal outcome for patient. Obtains and reviews referral information from nephrologists, dialysis units, or other referral sources. Ensures admission and pre-operative studies are completed and results provided to physicians. Facilitates clinical protocols for post transplant medication (immunosuppression, etc) management. Minimum Qualifications Bachelor’s Degree in Nursing 4 years Experience as a nurse in the medical, surgical, and or dialysis field. Demonstrated knowledge of current transplant-related medical information. RN - Licensed Registered Nurse_PA - State of Pennsylvania Upon Hire Preferred Qualifications Master’s Degree in Nursing Physical Demands Lift and carry 40-50 lbs. Examples: Push/pull patients on bed, stretcher (requires 29 lbs. push force), lateral transfers up to 50 lbs. of the patient's weight. Frequent to continuous standing/walking. Patient transporters can walk 8-10 miles per shift. *Patient care providers may be required to perform activities specific to their role including kneeling, bending, squatting and performing CPR.
