Chesapeake, Virginia Opportunity of a life time, practicing within an oncology office. The pharmacist will provide oversight of dispensing oral oncology and supportive medications at the onsite pharmacy. This setting will allow for patient pharmacist relationships to be established and provider collaboration. The pharmacist will utilize counseling skills, patient assistance knowledge, and provide comprehensive care. No drive thru, No storefront, No vaccines! Monday - Friday schedule - closed on major holidays Overview The Pharmacist Specialist is responsible and accountable for the provision of safe, effective, and prompt specialty medication therapies, encompassing daily dispensing, medication therapy management, patient advocacy and therapy compliance. The Pharmacist specialist provides required oversight for pharmacy technicians, including preparation and dispensing of drugs and clinical pharmacy services and will collaborate with providers, pharmacy, and nursing staff to ensure timely medication access. Additionally, the pharmacist specialist will oversee inventory management, staff development, and ensure compliance with accreditation standards, state and federal laws and regulations. What you will do Assesses, implements, and evaluates pharmaceutical care that contributes to the physiological and psychosocial outcomes and stability of the patient. Monitors, screens, and verifies all specialty medication and chemotherapy treatment plans for accurate dose, indication, route, treatment parameters, and administrative schedule. Completes critical patient monitoring and reviews patient profile/chart to identify, prevent, or mitigate drug-related problems, improper drug, or dose selection, sub therapeutic dosage, over dosage, adverse drug reactions, failure to receive drugs, untreated indications, treatment failures, and patient adherence issues. Provides drug information for patients and team members. Participates patient education related to pharmaceutical care and disease state management. Monitors patient progress, reconciles data discrepancies, handles specialty pharmacy inquiries, provides departmental updates, and promotes services to RMG practices. Provide statistical data related to specialty pharmacy capture rate and leakage of prescriptions from the RMG practice sites. Coordinates and processes copay requests including but not limited to patient status requests, copay assistance, prior authorization and billing inquiries, resolves payment obstacles, submission of appeal letters, eligibility inquiries, and claims processing challenges. Documents appropriately and timely within the electronic medical record or data analytics programs. Assists with program development and implementation to support system wide initiatives. Responsible for data governance of quality measures. Verifies data definitions, sources, and insures integrity of data collection. Responsible for transmission within RHS, manufacturers, and other external databases. Accurately and timely submission of data and information for required accreditation programs. Assures accurate and timely submission of validation records, data, and information. Handles all aspects of pharmaceutical care within the assigned location including supervision of pharmacy technicians. Participation in the development and training of clinical staff. Triages and/or approve referrals to specialty pharmacies through coordination of data/ prescription transfers with specialty pharmacies to ensure warm hand-offs of patients. Evaluates products and care plans to determine quality and cost-effectiveness. Assists with ordering and inventory control. Ensure compliance with the 340B program. Proactively seeks out pipeline medications, works with manufacturers and the wholesaler to gain drug access. Qualifications Education Doctoral Degree, Doctorate of Pharmacy (Required) or Bachelors Degree, Pharmacy (Required) Program Graduate, PGY-1 (Preferred) Experience 1 year Working with Specialty Pharmacy (Preferred) Licenses and Certifications Licensed Pharmacist - Virginia Department of Health Professions (VDHP) Licensed in the State of Virginia Upon Hire(Required) and Certified Specialty Pharmacist (CSP) - The Specialty Pharmacy Certification Board (SPCB) within 5 years (Required) Valid Drivers License Required To learn more about being a team member with Riverside Health System visit us at https://www.riversideonline.com/careers.
