JOB TITLE: Case Manager
JOB PURPOSE:To work with current and/or former shelter residents to develop and implement realistic plans that will remedy their homeless situation. EOE
ACCOUNTABILITY: Reports to Program Director
STATUS: Exempt
DUTIES AND RESPONSIBILITES:
1. Be committed to and follow the YWCA of Charleston Employee Handbook as well as the Rules, Procedures and Protocols governing the YWCA Sojourner’s Shelter Program including, but not limited to, maintenance of professional boundaries with the clients and residents.
2. Perform an intake within 8-24 hours to include administrating an assessment SPDAT
3. Facilities participants’ access to the system by being receptive and by arranging appointments as quickly as possible when entering the program
4. Assess economic, social, educational and mental health needs to circumstance of participant
5. Assist in the development and monitoring of social service plans with resident to achieve short and long-term self sufficiency goals
6. Assess participants’ strengths and needs to determine their eligibility for services and financial resources
7. Collaborate with professionals from different disciplines that may be required to ensure clients’ goal are met
8. Perform assessment to determine coping capabilities, potential support systems and areas of needs
9. Planning intervention and/or identify mainstream resources needed
10. Maintain accurate up-to-date records daily of participants’ progress, data utilizing HMIS system
11. Provide as much support as needed to participant in carrying out their social service plan
12. Maintain written statistical information as requested by Program Director
13. Communicate with Team Members, Assistant Director and Director on progress of assigned participants
14. Contact and advocate for participants with other social service agencies and community organizations
15. Meet with liaison from Department of Health and Human services as requested
16. Maintain a completed check list of required activities in each case file and participate in bi-weekly case review
17. Maintain necessary data, provide reports according to program/grant requirements
18. Attend and participate in HMIS and other assigned trainings
19. Meet with assigned participants once a week and more if needed to assess progress of their service plan
20. Other duties as assigned
QUALIFICATIONS:
1. BA/BS and/or maintain certification or eligible for licensure as a Social Worker or human services field
2. Minimum of 2-years experience in relevant field
3. Knowledge of federal and state laws and regulations pertaining to the homeless population
4. Knowledge of community resources available to assist the homeless population
5. Knowledge of theories, practices and ethics in the social work profession
