What you will do:

As a Register Nurse (RN) Case Manager you will oversee and help manage the delivery of care from pre-admission to post-admission. You will help ensure residents and their families receive appropriate utilization of services and coordination of care.

Must haves:

• Successfully graduated from an approved and certified RN program
• Current and unencumbered license to practice as an RN in the state
• Ability to make quick judgements and work independently
• Must be knowledgeable of nursing practices, procedures, terminology, laws, regulations, and guidelines that pertain to long-term care

Would be great:

• 2 years’ experience as an RN
• Active patient or bedside care within the past three years
