GENERAL RESPONSIBILITIES:

While demonstrating excellent customer service, the Licensed Practical Nurse (LPN) in Addiction Services at Whitney Young Health (WYH) provides general nursing support in the Methadone Maintenance Treatment Program and the Primary Care Suite under the direction of the RN Supervisor.

SPECIFIC RESPONSIBLITIES:

Age Specific Competencies for LPN :

• Demonstrates knowledge, skills and abilities to provide care to the age groups served.
• Interpret age-specific responses to treatment.
• Demonstrate knowledge of age-specific safety precautions.

Operations/Planning responsibilities for LPN:

• Review schedules and prepare daily activities as appropriate.
• Administer methadone adhering to the medication administration policy.
• Conducts drug screenings.
• Administers PPD Tests.
• Demonstrates knowledge of current immunization practices.
• Sets up appropriate work area including exam rooms.
• Performs appropriate nursing intake based on program requirements.
• Takes and records accurate vital signs.
• Accurately collects and records patient data.
• Maintains appropriate medical records utilizing the program’s medical record system.
• Maintains accurate patient telephone and written correspondence.
• Accurately completes charting, referral, lab and other documentation.
• Maintains inventory of medication and supplies, consults with RN regarding reordering.
• Communicates treatment changes with the treatment team to ensure comprehensive care.
• Coordinate patient care, provides recommendations and referrals to Health Center and/or community agencies as indicated.
• Completes required Health Center, State and Federal reports as requested by Supervising RN.
• Attends and participates in staff meetings.
• Attends case reviews and case conference meetings.

Miscellaneous:

• Must be CPR certified.
• Demonstrates excellence in both internal and external customer service.
• Understands and is able to effectively communicate HIPAA compliance, corporate compliance and client confidentiality.
• Ensures and/or remains in compliance with local, state, and federal regulation, i.e. JCAHO, OASAS, NYS DOH.
• Adheres to the National Patient Safety Goals as defined by the Joint Commission and the Whitney M. Young Jr. Health Center, Inc..
• Completes other duties as assigned.

Requirements

MINIMUM QUALIFICATIONS:

LPN candidates must have High School Diploma or Equivalent and be a graduate of a registered approved program for Licensed Practical Nurses with current New York State registration. LPN candidates must be able to demonstrate excellent customer service, and must have good communication and interpersonal skills.

CPR Certification.

PREFERRED QUALIFICATIONS:

One (1) to two (2) years physician office experience. Basic computer keyboard knowledge. Flexibility to adjust to schedule changes. Training in laboratory/phlebotomy techniques. Knowledge of managed care requirements.
