Care Coordinator will assess the needs of seniors and to develope a care plan to meet those needs. Includes follow up and monitoring the care plan. Completes a comprehensive assessment and completes determination of need for in-home services. Also investigates abuse, neglect and exploitation of seniors and anyone over 18 with a disability. Also follows up on those care plans. Also completes prescreens before clients enter the nursing home. Staff cars are supplied for out of town travel.
Care Coordinators will attend state training for the Abuse Program and for the in home service program.
A Care Coordinator needs to be very organized and have good people skills.
Must have a Bachelors degree in Social Work, Human Services or related field, or a RN or LPN
