Responsibilities
The Breast Health Nurse Navigator identifies patients who are appropriate for and served by the Comprehensive Breast Health Program, engages those patients in clinic services, and assists with coordination of care between services and across the continuum of care. The Nurse Navigator works collaboratively with multidisciplinary teams in both the inpatient and outpatient settings to ensure timely and effective access to care by facilitating referrals throughout the breast health program. The Nurse Navigator assists with the provision of care within the program as well as via telephone and virtual visits. The Nurse Navigator serves as a resource to patients, providers, departments within the WellStar system as well as external agencies.
