The Home Health Physical Therapist uses an individualized patient care plan to provide top quality care to all of our patients; keeping them Happy, Healthy and at Home. We are seeking a passionate Physical Therapist for our weekday team in Horry County This position is Full-Time Monday- Friday 8am-5pm. No on call responsibilities! Physical Therapists visit 5-6 patients on any given shift, driving to the patients home providing skilled therapy services. This position is primarily responsible for the initial assessment and case development of patients in all stages of life in their homes. Duties include but not limited to, assessing, interpreting, planning, implementing and evaluating patients according to the patient’s age and diagnosis.
