As a Physical Therapist at VNA Health you will provide excellent rehabilitative care to patients under the supervision of a supportive clinical management team.
As a PT, you will help develop and implement the plan of care, assess patient needs, provide treatments, educate patients, family members and caregivers re: use of appliances and equipment, and supervise Physical Therapy Assistants and Home Health Aides, as needed. As a Home Health PT, you will meet with patients where they reside, and work effectively with the other members of your interdisciplinary care team to achieve positive patient care outcomes. You will have the opportunity to provide in-service trainings to staff as well as meet with other PTs for peer discussion / learning.
