JOB DESCRIPTION SUMMARY

As a Physical Therapist at VNA Health you will provide excellent rehabilitative care to patients under the supervision of a supportive clinical management team.
As a PT, you will help develop and implement the plan of care, assess patient needs, provide treatments, educate patients, family members and caregivers re: use of appliances and equipment, and supervise Physical Therapy Assistants and Home Health Aides, as needed. As a Home Health PT, you will meet with patients where they reside, and work effectively with the other members of your interdisciplinary care team to achieve positive patient care outcomes. You will have the opportunity to provide in-service trainings to staff as well as meet with other PTs for peer discussion / learning.

POSITION QUALIFICATIONS
- DPT or masters degree in physical therapy from a nationally accredited university
- Must be licensed to practice as a physical therapist in California
- Minimum three years of relevant experience as a physical therapist required. Community / home health experience is preferred
- Excellent problem solving and critical thinking skills.
- Good verbal and written communication and strong organization & time management skills.
- Current CPR (BLS) certification
- Clean driving record and reliable, insured automobile
PHYSICAL REQUIREMENTS
- Ability to lift 25 lbs
- Ability to push/pull 200 lbs
- Ability to stoop, bend, and squat
- Ability to walk up one flight of stairs
- Ability to walk one mile
- Ability to sit for one hour, minimum
- Visual and auditory acuity
