Summary

The goal of the UT Southwestern Palliative and Supportive Care Team is to help patients live with the best quality of life possible by providing symptom management and psychosocial support. We are also dedicated to training the next generation of outstanding Academic Palliative Medicine Physicians with the skills to conduct clinical research, teach the next generation of residents and fellows, and provide exemplary care to patients from all backgrounds, especially those from underserved and culturally diverse populations.

Experience and Education

• Bachelor’s Degree in Nursing is required. 
• Relevant Case Management experience is required. Experience in Palliative Care is preferred.
• Licensed as Registered Nurse in Texas
• Basic Life Support (BLS) by the American Heart Association (AHA) or the American Red Cross (ARC)
• Recent experience in acute care setting and minimum 2-3 years’ experience in palliative care and/or end of life care. Oncology experience preferred.
• Knowledge of end of life care, including advance care planning, symptom management, and hospice.

Job Duties:

1. 1. Assists with the planning, coordination, implementation and education of UT Southwestern’s Palliative Care Program
      2. Participates and assists in palliative care screenings.
      3. Assists in clinical triage of new patient consults for the inpatient consultation service and ensures timely completion of consults by the palliative care team.
      4. Assists team with telephone calls and triage to provide safe, effective and appropriate disposition of acute problems using approved practice protocols (includes patient assessment, education, and crisis intervention).
      5. Collaborates with and coordinates care with referring teams and hospital nursing staff, as well as other multidisciplinary providers such as social work and care coordination. Participates as a member of the Interdisciplinary Team which delivers palliative care to patients and their families across multiple settings (currently, hospital and clinic).
      7. Conducts and is responsible for ongoing nursing assessments of the palliative care patient across the continuum in relation to treatment plans, patient and family inquiries, symptom management, psycho-social support, and education. May include initiating a plan of care based upon protocols or algorithms.
      8. Provides patient and family education for goals of care, end of life decision-making, home care planning, and hospice.
      9. Provides patient and family supportive services including legacy work, memory making, and coordination of referrals with community resources
      10. Assists with coordination of the palliative care volunteer program for the inpatient palliative care consultation service
      11. Assists in all facets of care coordination for palliative care referrals, including transition into outpatient palliative care clinics and hospice.
      12. Develops, plans and coordinates patient, staff, and community education efforts regarding palliative care and end of life issues.
      13. Develops and maintains policies and procedures; assists in the marketing of the program.
      14. Utilizes quality improvement process to improve effectiveness of care.
