Summary/ Objective: 

The Top 5% of the Medicare Advantage population is the highest risk and highest cost segment that consumes upwards of 50% of all utilization. The Extensivist Care Program creates an environment for regular monitoring and interventions for the various conditions the Patient may have. The Extensivist APRN will practice under protocols established by the Extensivist Physician. 

Essential Functions:

  • Performs health assessments and preventative health measures within prescribed guidelines, protocols, and Extensivist Physician instructions
  • Orders, interprets, and evaluates diagnostic tests to identify and assess Patient clinical problems and health care needs
  • Ability to perform basic clinical procedures such as Phlebotomy, IV insertion, Wound care, Suturing, Splinting, EKG, administering medications, Incision and Drainage etc.
  • Discusses and reviews cases with Extensivist Physician
  • Formulates and documents Plan of Care in Electronic Medical Record (EMR)
  • Prescribes medication or other forms of treatment for a Patient's diseases and conditions
  • Renders provision of care either in a home setting or remotely including but not limited to:
    • Start an IV
    • Administer IM injection
    • Administer Medications
    • Phlebotomy / Lab draw
    • Vitals
    • Administer Vaccinations
    • Perform an EKG
    • Blood Glucose Monitor
    • INR Checks
    • Spirometry 
    • Breathing/Nebulizer Treatments
    • Ankle Brachial Index
    • Perform Incision & Drainage
    • ACP Discussion
    • Joint Injections (if trained)
    • Perform Suture Repair
    • Splinting 
  • Keeps PCP updated on Patient status and actions taken
  • Will collaborate with Specialists where clinically required
  • Participates in On-Call program which may include after-hours, weekends, holiday calls, and visitations

Supervisory Responsibility:

  • No direct reports
  • APRN to report to Practice Administrator (Operational/Administrative) and the Extensivist Physician (Clinical)

Work Environment:

  • The APRN would operate remotely with assigned visits to Patient homes as needed

Physical Demands:

  • Manage and coordinate inbound and outbound telehealth and in-person outreach
  • Ability to document in Electronic Database systems
  • This is an active role. Requires actively examining and treating Patients in their home
  • Frequent bending and lifting

Position Type/ Expected Hours of Work:

  • This is a full time position and core hours of work and days are Monday to Friday 8:00 a.m. - 5:00 p.m.
  • Extended hours to accommodate Patient urgent and emergent care needs will be expected

Travel:

  • Approximately 75% of travel is required to the Patient home
  • Quarterly visits to Corporate/Regional office in Florida may be required

Required Education/ Experience

  • MS in Nursing preferred
  • Minimum 1 year of experience with clinical emphasis for APRN, preferably in Geriatrics
  • Current unrestricted Florida RN license and Florida NP license
  • Must have a valid DEA license
  • Must possess current CPR certification (BLS, ACLS, and PALS)

Additional Eligibility and Qualifications:

  • Ability to use Electronic Medical Record (EMR) system to document Patient encounters
  • Knowledge of ICD-10 Coding and STAR Measures attainment
  • Effective communication and interpersonal skills
  • Valid Florida Driver's License
  • For PhyPartners business units, the satisfactory completion of a pre-placement Tuberculosis test is a requirement for this position
