Come join Village Health Partners as a Clinical Support Specialist! This position will allow you to play an integral role in our Quality Department.

The Clinical Support Specialist provides support for patients, providers, and clinical staff by tracking and managing preventative screening results and records. This person will work closely with specialty practices to obtain patient records and close quality gaps, He or she will compile reports through Leading Reach, a referral health system, to help track and locate patient records. This position requires heavy patient and provider contact by phone.

Essential Skills and Functions:

Clinical:

• Handle incoming inquiries from any team member (Providers, CMA’s and other staff) regarding obtaining medical records and closing quality gaps.
• Compile Reports using Leading Reach system and correspondence by fax and/or mail.
• Contact specialty practices and other healthcare providers to obtain screening results.
• Link procedure results to the EMR so they can be reviewed by a provider.
• Access hospital portals to obtain medical records post discharge.
• Attend meetings with team and outside vendors.
• Follow all clinical safety protocols and infection control procedures.
• Other administrative duties as needed.

Teamwork:

• Ability to work effectively with other team members including:
  • Site Directors
  • Providers
  • Back Office Medical Assistants
  • Care Coordinators
  • Care Coordinator Lead
• Communicate effectively with team members, patients, and patient family members
• Maintain confidentially of patient and personnel information

Qualifications

• Minimum 3 years experience in physician office setting or direct patient care clinic
• Excel experience a plus
• Excellent customer service and communication skills.
• Highly motivated and organized.
• Experience with basic desktop software, medical practice management, electronic medical record systems, Microsoft Word and Excel
• Knowledge of and adherence to HIPAA rules and regulations
• Knowledge of and adherence to Bloodborne Pathogen & OSHA Safety procedures
