Description
The medical social worker is responsible for the implementation of standards of care for medical social work services. This position requires residence in Tucson, AZ area.
RESPONSIBILITIES, MSW
Under the guidance and supervision of the Clinical Coordinator and/or Program Director, is responsible for the delivery of varied social work services to hospice patients and their families. The purpose of this position is to provide initial psychosocial evaluations, ongoing psychosocial counseling, direct casework services, bereavement services, and community education, outreach and referral.

· Maintains the standards of Social Work excellence in accordance with the principles set forth by the Arizona State Health Department, and agency policies and procedures with 100% compliance.
· Performs comprehensive initial psychosocial and bereavement evaluations and assists in the development and implementation of goal-directed family care plan, with 100% compliance.
· Conducts ongoing reassessments of patient/family needs and counseling as required.
· Provides short-term crisis intervention and individual or family counseling when indicated.
· Participates as a member of the Bereavement Team as assigned.
· Observes, evaluates, and brings to team conferences information regarding psychosocial, spiritual, physical, and financial conditions affecting the patient and family.
· Assumes the active role of advocate for the patient/family unit.
· Develops and maintains contact with appropriate community agencies and services in order to promote interagency cooperation and to facilitate related referrals and marketing.
· Documents comprehensive psychosocial information clearly and concisely in a timely manner.
· All patient/family visits, telephone contacts and referral actions are recorded in the Medical Record within a period of 24 hours, with 100% compliance
· Participates in Social Services department programs as assigned by departmental director (i.e. Art Therapy, Crisis Intervention/School Programs, Bereavement Groups, Volunteer Training and additional new programs).
· Participates in committees, task forces and agency events as required.
· Attends staff meetings, IDG Team and other meetings as assigned and appropriate.
· Assists with community outreach and admissions programs when necessary to meet census goals.
· Participates in on-call coverage with social services team.
· Attends at least six workshops and in-service programs during the fiscal year (prorated for part time and Per Diem employees).
· Participates in the orientation program as assigned.
· Adheres to all policies.
· Assumes responsibility for own personal and professional development and maintenance of state of the art skills in social work.
· Exhibits Hospice Philosophy in all job related roles.
· Participates in Total Quality Management as assigned.
· Conducts and develops in-service training programs for assisted living and nursing home community, including research projects pertinent to the field of social work as available.
· Maintains current knowledge and practice of infectious disease protocols.
· Masters of Social Work degree from an accredited educational institution.
· Minimum of five (5) years experience in a clinical work, preferably either in a healthcare or hospice setting.
· Demonstrated ability in casework, counseling and group work.
· Valid driver’s license, car insurance and car required.
