We are looking for a talented CLINICAL COORDINATOR, to be an integral part of our Valor HospiceCare team who can successfully deliver social work services to hospice patients and their families. This position will require travel in the Green Valley and Nogales areas.

The Clinical Coordinator is a licensed Registered Nurse that is responsible for ensuring that patient care is coordinated and managed appropriately. The Clinical Coordinator is responsible for ensuring that care and services are delivered appropriately. The Clinical Coordinator provides direct supervision of clinical personnel, the Volunteer Coordinator, Office Coordinator, and others as assigned. The Clinical Coordinator reports to the Program Director/QCE.

Requirements

  • Registered Nurse degree with applicable state licensure.
  • Minimum of two years nursing in a home health/hospice or public health setting preferred.
  • Demonstrated ability to manage all clinical aspects of terminal care with expertise in pain and symptom management.
  • Demonstrated skill in expertise in both written and oral communication, supervision of staff, documentation requirements, cost efficiencies, marketing referral development, service coordination and case management.
  • Demonstrated ability to execute an effective, common sense, supportive, sensitive approach in response to needs of staff and patient/family.
  • Demonstrated administrative ability including knowledge of health care policies and program responsibilities.
