Upstate Services Group is Hiring a Social Worker Near Cortland, NY
Full Time Social Worker postition and a local Rehab and Nursing Facility.
BROAD FUNCTIONS:
- Serves as case manager for a team of residents. Is responsible for ensuring the effective delivery of health care services for residents in a cost effective manner.
- Assumes primary accountability for ensuring resident, family, and insurance carrier satisfaction with all phases of care delivered.
- Serves as primary counselor for residents and families on treatment team.
- Monitors team members with respect to non-discipline specific team, team-related job responsibilities. May supervise rehabilitation counselor students or social workers.
QUALIFICATIONS:
- Master’s degree in Social Work with Certification in Social Work preferred; or Bachelor’s Degree in Social Work or related field with regular access through a contract to consult with a CSW.
- Minimum of two years experience as a social worker in healthcare setting; long term care experience preferred.
- Demonstrated abilities in oral and written communication.
