We are a physician practice caring for patients with chronic illnesses with an emphasis on diabetes and vascular disease who we treat through co-management with our specialists such as podiatrists, endocrinologists, wound care providers and vascular care providers and in collaboration with their PCP.

Our team provides an additional layer of clinical and psychosocial support for patients with chronic illnesses. Upperline's interdisciplinary team sees patients in the clinic, home, hospital and facilities and provides wrap-around services including 24/7 clinical support, remote patient monitoring, behavioral health telemedicine and other resources. We help provide relief from the burdensome symptoms of chronic illness, help patients and caregivers navigate the healthcare system, and guide patients through difficult treatment choices. Upperline Health is based out of Nashville, TN and has clinics in Alabama, California, Florida, Georgia, Indiana, Kentucky and Tennessee.

About our Advanced Practice Provider Role

Upperline Health is seeking an Advanced Practice Provider for primarily home-based care in the Atlanta area. In collaboration with Upperline’s local Lead Physician, APPs develop complex plans of care in accordance with the patient’s health status and overall goals and values; provide medical diagnostics and treatments in the home consistent with the APP’s scope of practice and the patient’s medical condition; educate patients and families on medication usage, diet and nutrition, and crisis anticipation and prevention; communicate medical information to the patient’s physicians and other providers as well as to Upperline’s Lead Physician and broader clinical team. APPs take part in weekly interdisciplinary team meetings, call schedules and scheduled in-service and education sessions. 

Ideal candidates will have experience working with Medicare patients. Most importantly, candidates must demonstrate compassion for patient needs, have great attention to detail and thrive in a collaborative environment.

Responsibilities of the Advanced Practice Provider

  • Develop and implement clinical plans of care for adult patients facing chronic conditions (e.g., diabetes, COPD, CHF, debility, dementia)
  • Gather history and physical exam and diagnostics as needed in the home setting. Then develop and implement treatment plans given patient’s goals of care and current conditions
  • Meet patient and family physical and psychosocial needs with support and input from interdisciplinary team
  • Educate patients and families about medication usage, side effects, illness progression, diet and nutrition, medical adherence and crisis anticipation and prevention
  • Communicate with other Upperline clinical team members, patient’s other physicians and patient’s other medical providers to coordinate optimal care and resources for the patient and his or her family
  • Communicate with Upperline’s local Lead Physician to review patient records and care plans on a timely basis consistent with state regulations and company standards and policy
  • Maintain patient medical records and medical documentation consistent with state regulations and company standards and policy
  • Participate in continuing education as required by the state Board of Nursing
  • Prescribe medication as permitted by the state Board of Nursing
  • Other related duties as deemed necessary

Qualifications of the Advanced Practice Provider

  • Family, Adult Nurse Practitioner or Physician Assistant with appropriate certification and licensure is required
  • Possession of DEA registration or eligibility
  • CPR certification preferred
  • Upperline’s APPs must have strong compassion for our patients and their families
  • Thrives in a fast-paced, demanding medical environment
  • Must have reliable automobile, valid driver’s license and minimum state required liability auto insurance. NPs travel in personal vehicle with reimbursement for mileage.
  • Excellent oral and written communication skills to ensure that each of your interactions with patients, family caregivers, clinicians, and team members is effective and meaningful.

Compensation

Compensation is commensurate to compensation for similar positions in the region and based on prior training and experience
