Uplift Home Care Aides and Assistants provide each client with routine daily nurse aide

or personal care and services in accordance with the patient's assessment and care plan,

and as may be directed by the Administrator or Supervisor. Aides and Assistants are

responsible for observing, reporting, and documenting observations and patient care

performed during each visit with a client. Tact, patience, and empathy for the needs of the

ill, injured, frail, and the impaired are essential for this key position.
