JOB SUMMARY/ESSENTIAL JOB FUNCTIONS: Under the general direction of the Assistant Vice Chancellor, the Case Manager and serves as an institutional touchpoint for the needs of students. This position assists in coordinating prevention, intervention, training, and support efforts across campus and in identifying and connecting with community systems to assist at risk students and students facing crises, life traumas, and other barriers that impede success.

DUTIES AND RESPONSIBILITIES:

  • Serves as a member of the Behavioral Intervention Team (CARE Team), provides overview of student cases, and serves as a recorder.
  • Maintains accurate electronic case management records.
  • Works with a team to ensure continuity of care and coordinates and tracks referrals to internal units such as Student Academic Support Services and Inclusion, University Health Services, Student Conduct, Behavioral Intervention Team (CARE Team), and other University offices, as directed. May serve as a point-of-contact in triaging students to support services to support services and resources.
  • Compiles statistical data and prepares reports for program operation, activities and progress, as requested.
  • Designs and implements and develops individual action plans for identified students using evidence-based practices.
  • Compiles and maintains a directory of community resources and develop on-going referral relationships with community partners.
  • Facilitates broader resources linkages for students such as medication management and longer-term counseling services.
  • Supports students re-entering the University following Leave of Absences or Withdrawals.
  • Inputs, maintains and updates information regarding cases and inquiries in respective databases.
  • Coordinates student transitions to external, community providers (e.g. psychiatric/psychological treatment services, alcohol and drug treatment services, etc.)
  • Performs other related duties as assigned.

MINIMUM REQUIREMENTS:

EDUCATION: Master's degree in social work, counseling, higher education, student affairs, educational psychology, or related field.

EXPERIENCE: One-year related experience directly related to the duties and responsibilities specified. (Master’s Degree in a closely related field is preferred). (Training and experience with mental health systems and community resources in a college or university counseling center is preferred).

KNOWLEDGE, SKILLS, AND ABILITIES:

  • Experience and skill in presenting a wide range of student mental health concerns.
  • Successful ability to work with students from traditionally diverse groups.
  • Skill in designing individual action plans for distressed students, students with disciplinary or cautionary issues among campus resources, units, offices, and outside agencies, as appropriate, to provide quality intervention approaches to support student success and learning.
  • Ability to possess effective oral and written communication skills, with the ability to work effectively with a broad range of people including faculty, staff, students, health care professionals and members of the public.
  • Knowledge and understanding of treatment programs, mental health terminology, medical privacy guidelines, and ability to communicate difficult/sensitive information.
  • Skill in providing consultations regarding psychological development and behavioral health issues.
  • Skill in management of behavioral cases, student advocacy, and preventive approaches and appropriate responses.
  • Skill in critical thinking to anticipate potential problems or issues.
  • Demonstrated ability to commit to equity and inclusiveness in working with students from diverse groups.
  • Skill in applying best practices in developing and supporting equity and inclusivity.
  • Ability to provide excellent human relations and communication skills showing ability to work with diverse populations.
  • Ability to be flexible and willing to participate in office training and professional development. Ability to perform effectively in stressful situations.
  • Ability to work comfortably in a fast-paced environment and as a team member to support the office and the University’s mission and goals.

WORK SCHEDULE: This position may be required to work frequent evenings and occasional weekends.
