Job Summary

Responsible for the coordination of supportive services and patient care in a fast paced multidisciplinary clinical setting. Provides HIV related medical case management aimed at enhancing patient-centered care in coordination with physician and mid-level providers. Assesses and educates patients and medical providers regarding benefits available to patients through pharmaceutical companies and resulting eligible services. Provides assessment, intervention, and stabilization of the patient’s medical and psychosocial care needs via a thorough assessment of needs and a comprehensive service and referral plan. Provides advocacy, support and empowerment to clients as they navigate systems needed to support their ability to maintain access to HIV treatment and wrap around services.

Responsibilities

  1. Conducts comprehensive assessment of psychosocial and medical care needs of patients.

  2. Develops individual treatment plans through collaboration with the patient, medical providers, and community services with objective of creating stability and removing barriers to ongoing medical care.

  3. Provides information and referrals for community resources to patients including but not limited to housing, transportation , financial assistance, food, clothing, legal, mental health and substance abuse treatment.

  4. Assesses and discusses funding issues with patient, family, and health care providers to enhance cost-effective utilization of services and quality of care as well as negotiating with third-party entities to obtain authorization for services.

  5. Monitors quality of care and provides advocacy to patients in order to insure patient treatment plan goals are appropriate and they are understood and implemented. Routinely assesses patient response to services, while also measuring treatment plan effectiveness and necessity. Also identifies patient needs, including those of an ethical and cultural nature and insures that they are addressed.

  6. Provides timely documentation in database and patients’ medical record including interactions, interventions, and community resource contacts and communicates encounters to physicians and mid-level providers.

  7. Regularly attends meetings with clinical and community care teams for consultation and coordination of treatment planning needs.

  8. Manages care for a specific caseload of clients.

Problem solving. This position performs patient interactions with a confidential manner and provides community resource referrals on emergent and ongoing basis. Critical thinking skills are utilized to determine appropriate level of intervention and develop treatment plans which prioritize patient needs. Crisis intervention skills are utilized to defuse and assist individuals coping with traumatic situations.

This position develops knowledge of relevant community resource agencies and services for patient referral needs.

The knowledge of medical and psychosocial issues impact on individuals at risk for HIV is needed.

Minimum Qualifications

Bachelor’s degree in human or social services or equivalency (one year of education can be substituted for two years of related work experience). Demonstrated human relations and effective communication skills also required.

Fluency in the Spanish language is preferred.

This position is patient-sensitive and must fulfill all associated requirements. Preferences

SSW licensure, or Bachelor in Social Work, or Bachelor of Science in Nursing, or Bachelor in Health or Human Services strongly preferred. Experience working with high-acuity, complex patients including people suffering from chronic disease, mental illness, drug addiction and chronic homelessness. Reliable transportation. Fluency in Spanish.
