The Patient Care Coordinator and Scheduler performs a wide variety of duties associated with patient services. This position is responsible for all components of the patient experience ranging from patient in-take, office visit, patient close-out, and appointment scheduling. The position also oversees the reporting function for the clinic including compliance specific reports as well as corporate statistics.

Work Schedule: Monday to Friday 8:00am to 5:00pm 

Office Location: 4700 W Sam Houston Pkwy N, Ste 220 Houston, TX 77041

Primary Responsibilities:

  • Coordinates initial patient contact and ensures patient eligibility to clinic and verifies the appropriate primary care physician is listed prior to setting up appointments
  • Reviews quality detail report to determine which patients have outstanding measures and need to be scheduled
  • Scheduling of patients with notification to PCP group of patient list and location of the mobile unit
  • Prepares intake forms for healthcare screening clinics to ensure providers and staff have the most up to date forms available for patient visit
  • Requests medical records for appropriate use by providers to assess healthcare diagnosis code assessment
  • Coordinates all clinic schedules with PCP group and patients including scheduling transportation or other special services as indicated for the patient visit
  • Initiates Comfort Care appointments for patients as needed and trouble shoots problems that may arise between Comfort Care and the patient
  • Coordinates with PBMs and PCP groups for patients that are not reachable as well as involving management to take next steps to follow-up with the patient
  • Coordinates with the providers and contracted PCP groups and ensures a timely submission of the attestation sheets to the DataRap department
  • Audits completed screenings to ensure all information is completed and faxes all quality forms to DataRAP for processing
  • Reviews charts for outstanding referral services needed such as colon screening, DM eye exam, and mammograms to make sure patient has appointment within required timeframe
  • Validates patients’ attestation sheets and conducts research to determine if and when a patient may need a follow-up visit with their PCP as indicated in provider progress note

Required Qualifications:

  • High School Diploma/GED (or higher) 
  • 6 months of experience working in a medical office
  • 6 months of experience with strong communication and interpersonal skills, both in person and via phone
  • Intermediate level of proficiency with PC software computer skills such as Microsoft Word, Excel, Power Point, and other departmental specific programs/applications
  • Ability to work 40 hours a week
  • Valid driver's license and reliable transportation as travel to other clinics will be required

Preferred Qualifications:

  • Working knowledge of EPRG and Medicare Advantage quality requirements
  • Knowledge of medical terminology and anatomy
  • ICD10 and CPT coding experience
  • Must be able to prioritize, plan, and handle multiple tasks, demands simultaneously
