POSITION SUMMARY: The Case Manager will be under the direct supervision of the Program Director. The Case Manager will be responsible for providing comprehensive case management services including, but not limited to academic and credentialing supports, career counseling, personal guidance and development, life skills, referral services, and youth and family advocacy in a safe, caring, and enriching environment for the participants enrolled in the Train & Earn (T&E) program. The Case Manager will have the opportunity to work in both individual and large group settings with a vulnerable population through constant engagement, one-on-ones, workshops, and training.

The Case Manager is responsible for recruiting, enrolling, and establishing Individual Service Strategy plans for two cohorts per year. The 6-week education component includes a number of different workshops and activities focused on the development of employability skills and self-efficacy. During this time Case Managers will support participants’ training through workshop participation in collaboration with the Education team and job counseling in partnership with the Job Developer, as well as document progress towards established goals, maintain and submit individual participant case notes, and identify and coordinate support services for participants as needed. Following completion of the 6-week training, T&E provides participants wrap-around services for one year, including alumni workshops, career counseling, needed referrals, and mentorship. The Case Manager will support the coordination and execution of follow-up activities and retain all cohort participants in their caseload. The Case Manager will support and document the progress of those same youth throughout the one-year follow-up period to help navigate and advance their short- and long-term goals.

PRIMARY JOB RESPONSIBILITIES (include but are not limited to):

● Recruit program participants by leveraging a range of strategies and community-based partnerships

● Conduct intake interviews, in partnership with other members of the Train & Earn team, and associated paperwork to select and enroll participants for each cohort cycle

● Onboard participants and develop initial Individual Service Strategy (ISS) plans, reflecting SMART short- and long-term goals for each participant

● Maintain a caseload of 60 participants (30 base year and 30 follow up), including a biweekly check-in with each participant, making appropriate referrals to address barriers to employment, and regular reassessment of needs

● Provide individualized guidance for participants in their pursuit of college and/or career opportunities, including close collaboration with the T&E Job Developer, T&E

Education Specialist, and relevant community partners and resources

● Complete and submit a monthly case note for each participant, per DYCD standards

● Collect, account for, and submit quarterly attainments for each participant, per DYCD standards

● Coordinate and collaborate with Director and Education Specialist to prepare the schedule, training, administration/proctoring, and validation of credentials to be achieved through program activities

● Coordinate and collaborate with the Director and Education Specialist to prepare the scheduling, academic support, administration/proctoring, and validation of TABE testing and measurable gains

● Co-facilitate weekly full-staff case meetings to support regular sharing of up-to-date, relevant information on participants and their progress and ensure ISS plans are adhered to or revised as necessary/appropriate

● Support T&E Director and Job Developer by maintaining positive working relationships with social services agencies, CBOs, and other community partners to enrich programming and internship/employment opportunities in alignment with participants’ interests and ISSs

● Work with Job Developer to build and retain pipelines for employment, especially in consideration of participants’ skills and interests

● Collaborate with Education Specialist to support alignment of content with participants’ interests, needs, and proficiency levels while ensuring all DYCD deliverables and validation documents are accounted for.

● Collaborate with Education Specialist to prepare and deliver meaningful enrichment content

● Coordinate and collaborate with Education Specialist to develop and deliver engaging alumni workshops, and execute other retention activities

● Collaborate and coordinate with community partners for referrals and other specialized services to deliver comprehensive wrap-around services that address participants’ needs

● Advocate for and coordinate access to services, resources, and opportunities on behalf of participants, including needs for child care, housing, justice involvement, substance abuse, violence prevention, and/or access to public assistance benefits

● Maintain a positive rapport and regular communication with participants for provision of active/ongoing wrap-around services and continued counseling/mentorship as youth advance through the program

● Collect, maintain, and audit participant files for completion and accuracy in accordance to DYCD and UAU standards and WIOA privacy guidelines

● Develop and maintain strong working relationships with staff, youth, and program stakeholders

● Participate in team and community partner meetings and events, and professional developments

● Assist with any other departmental duties and projects as needed

JOB QUALIFICATIONS:

● Associates Degree or higher from an accredited institution

● Minimum 2 years of experience working amongst individuals between the ages of 16 to 24 years of age

● Minimum of 1 year of case management experience

● Reliable mode of transportation

● Valid driver’s license

● Strong verbal and written communication skills

● Excellent organizational and time management skills

● Demonstrated ability to conduct and complete work independently

● Willing to take initiative and be flexible when needed

● Demonstrates the ability to connect with and maintain positive relationships with vulnerable populations

● Successful experience networking and providing/securing referrals and resources

● Values individuals’ different capacities and abilities

● Ability to conduct and complete work independently

● Excels working in a collaborative environment to achieve target objects and outcomes

  • Involves sitting approximately 50 percent of the day, walking or standing the remainder.
  • Must be able to remain in a stationary position for at least 50% of the time.
  • May include working prolonged periods of time sitting at a desk and working on a computer.
  • The ability to communicate information and ideas so others will understand. Must be able to exchange accurate information in these situations.
  • The ability to observe details at close range (within a few feet or more of the observer).
  • Ability to life 30-50lbs as needed.
