We are pleased to announce the following available position: FT DISCHARGE PLANNER
The Discharge Planner is responsible for scheduling aftercare appointments, interfacing with the treatment team/managed care organizations as needed to identify and arrange next care provider appointments and collaborating with next level of care professionals for continuity of care. Also ensures that all involuntary admission paperwork is filed with the appropriate court office and proper documents are forwarded to the guardian or patient’s legal representative within the prescribed timeline.
QualificationsQualified candidates must have a minimum of a Bachelor’s Degree in a related behavioral health services field with a minimum of 1 year case management, patient care or discharge planning experience in an inpatient addiction/mental health facility. Must be proficient
