Granite Hills Hospital offers high-quality, patient centered care for the greater Milwaukee community. With a wide range of treatment for children, adolescents, adults and older adults, our mission is to provide much needed outcome-focused care for those who need it most. Our new 120-bed facility features evidence-based treatment modalities and a multi-disciplinary team led by expert medical professionals. The 120 beds are split into 5 groups of 24 with enclosed outdoor spaces to promote healing.

We are looking for a Step-Down Navigator to join our new team. 

The Step-Down Navigator is responsible for meeting with all eligible patients to discuss the benefits of Outpatient services as part of their continuum of care treatment. The Step-Down Navigator will be responsible for communicating eligibility to key members of senior leadership & associated departments and will be responsible for routinely collecting and entering data, specific to Outpatient eligibility, into Midas for trending report purposes. The Step-Down Navigator will have a thorough understanding of the Step-Down Playbook, and implementing components of the Playbook as needed. A thorough understanding of the Inpatient workflow process, as it pertains to case management and discharge planning, is preferred.

Qualifications

Education: Bachelor’s Degree in mental health/behavioral sciences field preferred.

Experience: Minimum two (2) years’ experience in psychiatric/mental health field.

Licensure: N/A
