The Clinical Social Worker assists in the integration of social services functions in the patient care, discharge planning and aftercare planning process with other hospital departments, external service organizations, agencies and health care facilities. The Clinical Social Worker provides direct care to patients as appropriate and ensures compliance with quality patient care and regulatory standards. The Clinical Social Worker assists patients and their families using psychotherapeutic skills to understand, identify and resolve complex problems resulting from psychiatric illnesses utilizing group and individual therapies. Duties include but are not limited to:

  • Conduct psychosocial assessments.
  • Provide social work consultation to physician, nurses, other hospital staff members, patients, patient’s relatives and coordinates with community agencies upon requests and in coordination with assigned social worker.
  • Provide discharge-planning services and arrange for post-hospital/post-outpatient treatment care of patients through external services as requested.
  • Coordinates social services discharge and home care planning processes to facilitate continuity of care with all assigned patients.
  • Lead and/or co-lead group therapy on assigned hospital units and outpatient services.
  • Demonstrates ability to conduct family and community interventions appropriately.
  • Coordinates social services treatment plan with patient treatment plan within appropriate timeframe.
  • As requested, assists patients and families in understanding, accepting and following medical recommendations.
  • Completes reporting (child abuse, adult abuse, Tarasoff, domestic violence) activities per Del Amo Hospital policy.
  • Always maintains complete thorough records, including charting. Reviews and updates periodically.
  • Provides information and guidance to patients, families and other hospital departments regarding psychosocial aspects of patient’s post-discharge treatment and recovery plans as requested

Qualifications

Education: Master Degree in Social Work, Counseling, Psychology or related field required.

Credentials: Licensure by the California Board of Behavioral Sciences as an LCSW, MFT or Registered Associate/Intern preferred.

Experience: Previous placement or internship in a psychiatric setting preferred.

Knowledge/Skills:

  • Must be able to perform assignments with minimal supervision;
  • Must be able to make sound, independent judgments based on scientific and/or ethical principles;
  • Must be able to collaborate with other multidisciplinary team members in an appropriate fashion;
  • Must be capable of adapting to varying workloads an work assignments on a constant basis;
  • Must have effective comprehensive reading skills, strong communication skills, written and verbal.
