UHS of Delaware, Inc. is Hiring a LEAD CASE MANAGER Near KIRKLAND, WA

Responsibilities

Working as a Case Manager at Fairfax allows you to collaborate and engage with a multidisciplinary team while working with an acute population. You will work with patients to assess their psychosocial needs, identify discharge plans, and facilitate 1:1 meetings patients, as well as family meetings, to support patients in achieving an optimal level of emotional health. The Case Manager will work under the guidance of a Licensed Social Worker to meet all regulatory standards and requirements. We are looking for compassionate and skilled individuals to bring additional expertise and experience to our team. Use your therapeutic skills to watch the growth of patients and make a lasting impact. The Lead Case Manager assists the Managerin training and administrative duties as assigned.

Qualifications

QUALIFICATIONS Bachelor’s degree from an accredited college or university, Master’s degree preferred; mental health field experience required.  Agency Affiliated licensure required (can be obtained upon hire)  2 years post-Bachelor’s experience in a psychiatric health care setting preferred, not required  Direct experience with crisis intervention, case conceptualization, and discharge planning.  Strong clinical assessment and case conceptualization skills required. Employee is required to obtain CPR, First Aid, Handle with Care, and Verbal De-Escalationcertifications within 30 days of hire.

*For those with Master’s level associate licensure, Fairfax may provide supervision of hours to help our employees achieve their goal of independent licensure.*

RESPONSIBILITIES: Assess and identify psychosocial, educational, developmental, and behavioral functioning of the patient/family system utilizing evidence-based practice. Facilitate 1:1 sessions with patients to collaborate with their treatment and discharge planning Consult with families on emotional, social, and financial aspects of the patient’s life and facilitate family meetings for patients (when warranted) Evaluate and document effectiveness of interventions and patient response to treatment Act as liaison between payers, the business office, and treatment team, providing information and feedback to assist in optimum patient care and reimbursement. Comply with required standards for medical/legal reporting situations (i.e. child abuse, dependent adult abuse, duty to warn, etc.) Have a role in multidisciplinary meetings, patient care conferences, and treatment planning Participate in performance improvement projects and activities Continued professional development and training Maintain flexibility in regard to assigned unit and complete other duties as assigned

KEY SKILLS AND ABILITIES: Strong clinical assessment skills Strong clinical skills in meeting with patients and/or families Ability to create effective working relationships with team, patient and/or patient family Ability to recognize the importance of adapting to various patient demographics Effective oral and written communication skills Ability to accept direction, adhere to organization policies and procedures, and adapt to changing policies and procedures Strong planning, organizing, time management, and computer skills Proficiency in record maintenance, reporting, and correspondence Effective interpersonal skills; must be customer-service oriented, show reliability and initiative, and demonstrate tact, resourcefulness, patience, and dedication

PHYSICAL, MENTAL, AND SPECIAL DEMANDS: Ability to stand, or walk up to six hours, sit for up to two hours. Ability to frequently lift and carry up to 10 lbs. (charts, records). Ability to discern a variety of odors. Ability to handle hazardous/infectious waste on an occasional basis. Ability to work in an environment where the noise level is frequently high, where chemicals are frequently used for cleaning; where mechanical and electrical hazards may occasionally be present; and where dust, mist, and steam are frequently generated in housekeeping tasks. Ability and knowledge of how to respond when exposed to blood and bodily fluids. Ability to participate in the management of acutely agitated patients consistent with Handle with Care and hospital procedures.
