Responsibilities

Canyon Ridge Hospital is a leading Behavioral Health Facility located in Chino California. The hospital is part of the Universal Health Services Network of Behavioral health facilities. Our Mission is, “To be a leader in behavioral healthcare, dedicated to excellent service in the community.” This is a Per Diem- PM Shift Position

The HIM Tech supports the Health Information Management Department through variety of clerical, technical, and related support services.

Essential Job Duties:

  • Compiles and maintains daily and monthly statistical data in a timely and accurate manner:
  • Reconciles daily census and communicates census numbers as requested.
  • Provides discharge information for use at the Daily Case Management Meetings.
  • Generates and distributes census reports as directed.
  • Processes admissions on a daily basis:
  • Maintains the integrity of the Master Patient Index.
  • Processes discharges on a daily basis:
  • Maintains the integrity of the Master patient Index.
  • Creates new folders or updates previous folders.
  • Maintains system for identifying individuals with more than one medical record number or medical record numbers applied to more than one patient.
  • Maintains procedures for selecting the surviving medical record number and merging duplicate numbers and files
  • Maintains communication system notifying other departments of duplicates and surviving numbers.
  • Monitors the accuracy and integrity of electronic and manual merging of duplicates.
  • Updates the dry-erase board by erasing records returned to Health Information Management.
  • Assembles discharged patient records in accordance to departmental policy and procedure.
  • Analyzes discharge patient records according to specified guidelines.
  • Codes patient records within specified time frames, as needed, for billing purposes.
  • Abstracts the medical record according to OSHPD guidelines;
  • Abstracts every inpatient discharge for the semi-annual report to OSHPD.
  • Corrects errors to data reported in the test mode to OSHPD to ensure data accuracy and ensure certain data is below the error tolerance rate in order to submit formally.
  • Monitors dictation/transcription of Psychiatric Evaluations and History & Physical Reports for timeliness and completeness.
  • Processes correspondence/requests for information on an as needed basis to cover for HIM Clerk:
  • Demonstrates a complete understanding of the release of information process, including guidelines in regulatory manuals and facility policies and procedures.
  • Handles requests and inquiries for patient information; dispersing the information with accountability to all regulatory entities and according to the facility’s policies and procedures.
  • Copies and mails/faxes records for requests needed immediately.
  • Reviews incoming requests, pulls charts, and either prepares records for copying by service or makes and mails copies, as appropriate.
  • Copies and distributes records to insure compliance with Business Office, Utilization Review, and/or managed care policies and procedures.
  • Maintains the Correspondence Log Book in an accurate and timely manner.
  • Communicates effectively at all levels internally and externally by expressing ideas and information clearly and concisely, in verbal and written form.
  • Answers the telephone and provides assistance or receives messages, as appropriate.
  • Processes, distributes and/or files transcribed reports of both in-house and discharged patients as needed in the absence of HIM Clerk.
  • Accurately files all loose reports of discharged patients.
  • Maintains both the permanent and incomplete files in a neat and accurate manner.
  • Clears charts after Clinician completion and maintains the Clinician deficiency file in an accurate and timely manner.
  • Pulls charts as requested for chart review, audit and completion.
  • Files charts accurately.
  • Purges inactive medical records as defined by departmental policy.
  • Requests stored records from outside storage-company in accordance to facility’s policy and procedures.
  • Provides back-up coverage for PBX as required for meal and rest breaks.
  • Maintains the confidentiality of all patients, Professional Staff, and hospital-related issues, at all times, on and off duty.
  • Performs other duties relating to the work of the Department as required or assigned by the Health Information Director.

Qualifications

Previous Health Information Management or related experience required; typing, transcription, filing and record-keeping and record-coding skills; work organization, prioritization, and time management skills; sound judgment and decision-making skills; minimum of high school level reading and writing; familiar with general medical terminology needed with specific familiarity with psychiatric terminology helpful; experience with ICD- 10-CM procedures and use of DSM V coding procedures highly desirable; general clerical and office procedure skills; business correspondence and communication skills; skills in proper use of office equipment; experience in psychiatric facility helpful.
