The discharge planner is responsible for assisting in discharge planning coordination with unit, social worker and Utilization Review staff. This position entails attending treatment planning meetings, reviewing clinical documentation and seeking appropriate aftercare referrals.

Education: High school graduate, bachelors preferred. Completion of basic clerical or medical/clerical course is preferred.
Experience: A minimum of two (2) years’ experience in a clerical support role, preferably in a psychiatric health-care setting.
