Responsibilities

Cedar Hills Hospital is a 98-bed behavioral health facility in Portland, Oregon dedicating to healing our patients’ minds and bodies through a combination of high-quality clinical services and holistic healing practices.

We are seeking a Discharge Planner with an interest or background in behavioral health to join our team and to support our patients in one of our specialty care programs:

• Crisis Stabilization
• Substance Abuse
• The Military Program

The Discharge Planner advocates for patient access to services during treatment team meetings, helping to support and coordinate ongoing patient care and aftercare needs. The Discharge Planner contacts external case managers/managed care organizations, physicians, and other care supports throughout the patient’s stay to assist the treatment team in understanding the patient needs and respective insurance carrier requirements for discharge planning.

Responsibilities include but are not limited to:

• Promoting effective use of resources for patients
• Advocating that the patient is placed in the appropriate level of care and program
• Maintaining documentation related to case management activities
• Oversight of continued communication of treatment progress, discharge plans, and time of discharge to outside parties
• Facilitating the transfer of required documents to the outside agents

Qualifications

Requirements:

• Bachelors from an accredited college or university in social work, mental health, nursing or related field preferred.
• Experience in a healthcare setting or managed care company, with experience in patient assessment related to discharge planning, treatment planning, and/or case management preferred
• May be required to work occasional overtime and flexible hours
