UHS of Delaware, Inc. is Hiring a Case Manager Near WEST ALLIS, WI

Responsibilities

Granite Hills Hospital offers high-quality, patient centered behavioral health care for the greater Milwaukee community. Our 120-bed facility will feature evidence-based treatment for adolescents, adults, and older adults. 

We are looking for a Case Manager to join our new team!

The Case Manager is responsible for managing the treatment activities offered to the patient, which may include the appropriate clinical screening of potential admissions, establishment of treatment plans, interface with the treatment team/external case managers/managed care organizations and implementation and supervision and/or accomplishes all aspects of the discharge planning process. The Case Manager is responsible for performing the utilization review/management function in accordance with the facility’s Utilization Review and Management Plan. This includes, but is not limited to, completing admission and continued stay reviews within the required timelines and utilizing established admission and continued stay criterion as well as communicating findings of these reviews to the attending physician and other members of the treatment team as appropriate. The Case Manager maintains current knowledge of facility’s Utilization Review and Management Plan and established admission and continued stay medical necessity criterion as well as admission and continued stay medical necessity criterion which may be specific to various individual payers with whom the facility contracts.

Qualifications

Education: Bachelor, Masters, or Doctorate Degree from an accredited college or university in social work, mental health, psychology or a related field preferred.

Experience: Two (2) years direct clinical experience in a psychiatric or mental health setting preferred. Experience in patient assessment, motivational interviewing, treatment planning, and communication with community supports and resources is preferred. Strong written, verbal and interpersonal communication skills are required.

Additional Requirements: Successful completion of CPI and BLS training and certification. May be required to work overtime and flexible hours.
