UHS of Delaware, Inc. is Hiring a Care Manager Near LANCASTER, PA

Responsibilities

 Lancaster Behavioral Health Hospital (LBHH) is a newly opened (July, 2018) 126-bed, inpatient psychiatric hospital and start-up joint venture between Lancaster General Health/Penn Medicine and Universal Health Services.

In addition to a general adult unit and dedicated adolescent unit, the new facility will also offer services for treating those with co-occurring disorders on the psychiatric-medical unit, an intensive adult unit to care for those with severe conditions, as well as a women’s trauma unit.

Care Managers are responsible for completing the following duties and tasks:

  • Develops and coordinates individualized discharge plan for the patient by utilizing treatment team and written chart information to determine the patient’s aftercare needs. Accesses by phone or written correspondence family or community resources such as nursing homes, group homes, or private mental health practitioners to meet patients’ aftercare needs.
  • Coordinates with Utilization Review for aftercare appointment information and is familiar with Utilization Review process.
  • Completes discharge planning/continuing care form, including demographics section, follow-up appointments, and other treatment recommendations, if applicable.
  • Meet with patients upon admission to begin discussing discharge planning, obtaining any relevant information about prior/current MD’s or counselor’s patient may be seeing. Provides supporting counseling where indicated.
  • Assists Social Services in compiling accurate referral source and contact database.

Qualifications

What Does LBHH Require for this Role?

  • Master’s degree in mental health related field and/or equivalent combination of education and experience required.
  • State of Pennsylvania Social Worker License and 2 years of social work experience preferred.
