Responsibilities

Horizon Health, the nation’s leading psychiatric contract management services, in partnership with Good Samaritan Hospital in Vincennes, IN (close to the IL state line) is seeking a full-time Intake Services Coordinator for its 20-bed adult inpatient behavioral health unit.

The Intake Coordinator, in conjunction with the Program Director and Nursing Supervisor, coordinates the referral development and pre-admission patient assessment process, and provide clinical assessments for potential clients.

• Position responsibilities include but are not limited to:
• Conducts on site patient assessments in the Emergency Department, as needed
• Responds to intake calls and completes admission paperwork
• Coordinates treatment with physicians and other healthcare and social agencies
• Coordinates and participates as a direct liaison with appropriate interested parties and physicians through the patient referral inquiry and admission process
• Reviews newly admitted and recently discharged patient accounts.

Qualifications

Requirements:

• Registered Nurse (RN), or Master’s prepared Social Worker or Counselor (Licensed MSW or Counselor preferred)
• Must have at least two years’ experience in admissions, intake or marketing social services; preferably behavioral health/psychiatry.
