We are looking for a Case manager to enhance the quality of case management, while maximizing client satisfaction and maintaining caseload compliance. The successful candidate will be accountable for caseload management. Case manager duties include assessing, planning, implementing, monitoring and evaluating actions required to meet all compliance metrics. In person child visits occur one time per month.

Currently hiring for CPA Case Manager and for General Residential Operation Case Manager. CPA Case Manager works with foster families and children and General Residential Operation Case Managers work directly with children living on site on our property, The Village.

Responsibilities

• Handle case assignments, draft service plans, review case progress and determine case closure.
• Facilitate multiple care aspects (case coordination, information sharing, etc.)
• Help clients make informed decisions by acting as their advocate.
• Develop an effective working relationship and cooperate with supervisor throughout the entire case management process.
• Take the extra mile and interact with clients to keep track of their progress and to ensure satisfaction.
• Record case information and complete all necessary forms accurately.

Skills

• Proven working experience in case management, or a degree in a relatable field.
• Knowledge of case management principles.
• Effective communication skills.
• Excellent organizational and time management skills.
• Problem solving skills and ability to multi-task.
• BS degree in related field
