About the job:

Under direct supervision, is responsible for one-on-one continuous observation and surveillance of assigned patient. Remains with and observes assigned patient at all times and notifies the nursing staff if the patient requires additional assistance or other nursing intervention.

What you will do:

  • Demonstrates the knowledge and skills necessary to provide service/care appropriate to the age of the patients served, in accordance with MercyOne standards.

  • Receives shift change report from RN, previous Patient Safety Attendant and/or CNA regarding patients’ anticipated behavior and safety risks. Hands off information to the next shift RN, Patient Safety Attendant or CNA.

  • Introduces self to patients/families and explains that they are there to assist in maintaining a safe environment.

  • Remains with and observes the assigned patient at all times. Arranges meal and break time with the patient’s RN or CNA so that the patient is never unattended.

  • Serves meal to patients.

  • Provides follow-up to meal service; collects trays; records fluid intake, calorie counts and appetite assessment when specified; responds to patient concerns with appropriate action.

  • Utilizes the call light, immediately notifies nursing staff if the patient requires assistance, exhibits changes in behavior, or requires other nursing interventions.

  • Remains alert at all times.

  • Maintains a neat and orderly work environment. 

  • Communicates effectively with the Registered Nurse and other multidisciplinary team members as well as patients, families and significant others.

  • Observes patient in a 1:1 setting as assigned. Monitors patient in accordance with unit guidelines. Patient is not to be left alone.

  • Initiates appropriate measures to ensure the safety/well-being of patient, family, visitors and staff, i.e., side rails, call light within reach, bed alarm and personal alarms as needed.

  • Actively participates in intradepartmental meetings/in-services on unit and/or as provided by MercyOne Educational Services and committees as requested by nurse manager.

  • Recognizes and communicates issues/occurrences utilizing the VOICE documentation system and notification of nurse manager, as needed; assists in identifying ways to resolve issues and problem solving.

  • Is able to float within the patient care units by maintaining working knowledge of hospital-wide/unit policies and procedures.

  • Is knowledgeable of and actively participates in improving patient safety and reducing risk to patients. Complies with Safety and Infection Control Policy & Procedure.

  • Maintains and updates job competencies on an annual basis.

  • Knows, understands, incorporates, and demonstrates the MercyOne and Trinity Health Mission, Vision, and Values in behaviors, practices, and decisions.

  • Maintains a working knowledge of applicable Federal, State, and local laws and regulations, MercyOne and Trinity Health’s Organizational Integrity Program, Standards of Conduct, as well as other policies and procedures in order to ensure adherence in a manner that reflects honest, ethical, and professional behavior.

  • Maintains confidentiality of information pertaining to patients, physicians, employees, and MercyOne business.

  • Demonstrates general working knowledge of computers and department specific software (i.e.Healthstream, internet, etc.)

  • Performs other duties and responsibilities as assigned.

  • Remain calm in stressful situations to create a healing environment. Utilizes empathetic responses to reduce patient stress.

What we are looking for:

  • Completion of high school or GED is preferred but is required if in emergency department or behavioral unit. Prior patient care experience preferred.

  • Must be available to work on short notice.

  • Is able to communicate effectively with all members of the health care team.

  • Is able to perform a variety of duties characterized by frequent change.

  • Must maintain Mandatory Reporter, to be obtained within 3 months of employment.

  • Must maintain Violence Safety, to be obtained within 6 months of employment.
