Trinity Health Of New England is looking for an experienced Case Manager (RN) to join our team of mission-driven healthcare innovators at Saint Francis Hospital. This person serves as the driver of a collaborative, interdisciplinary process which coordinates, monitors, and plans health care services throughout an episode of illness.
The ideal candidate will display a drive to provide patient-centered experiences working as a resource and guide for Saint Francis physicians, the entire care delivery team, patients and their families throughout the continuum of available health care services. This position requires a person with a positive attitude who is self-directed, pleasant, cooperative, assertive yet compassionate displaying a professional demeanor with patients, families, physicians, and fellow employees.
Position will require flexibility to work at different campuses, includes on-call responsibility, and an overview of 24 hour care.

You Will Be Responsible For:
Exceptional Patient Care: The Case Manager assesses, plans, negotiates and procures the care/services for patients of all ages across the continuum to facilitate achievement of high quality, cost-effective patient outcomes.
Excellent Communicator: Communicate effectively (written and verbal) with admin, colleagues, patients and family member.
Strong Educator: Educate patients and family members so that they have a transparent understanding of the care being provided.
Technical Familiarity: Understanding in computer-based applications (E-MAR, clinical documentation), email, and standard applications such as Microsoft Office.
Active Teamwork: Ensures validity of accurate documentation of severity of illness (profiling) for the Saint Francis System by working with the physician in the concurrent review process.
Requirements:
Education:Graduate of an accredited School of Nursing.
Licensure: Current Licensure as a registered nurse (RN) in Connecticut
Experience: three years of diversified, progressive experience with at least 1 year of recent experience in an acute care facility or public health nurse setting.
Including an in-depth understanding of and skill in using many nationally recognized medical utilization criteria sets (i.e. MCG, InterQual, and others as needed).
Requires a solid understanding of managed care methodologies, HCFA/OBRA and state of Connecticut regulatory compliance issues, and JCAHO accreditation accountabilities of the Continuum of Care
Certification: BLS certification required
Ability to pass drug screenings and background checks
Preferred Skills:
Education: BSN preferred
Certification: Certification in Case Management (ACM, CCM) required within one year of employment after meeting minimum requirements.
