Description:

Completes various functions of medical record handling, such as chart

pick-up/retrieval, reconciliation, pull, sort and files medical records and tracking of medical records in the Meditech system.

Maintains and operate a variety of health record databases and HIM-related applications to collect, classify and analyze information. Use computer programs to tabulate and analyze data to improve patient care, better control costs, provide documentation for use in legal actions, or use in research studies. May process Birth Certificates and related document, if assigned.

Processes STAT requests for release of PHI information in a timely manner according to HIPAA regulations.

Receives, processes, and files health information to ensure completeness, timeliness, and accuracy of the medical records. Preps charts for scanning, scans charts and loose documents, and indexes charts into Meditech system. Performs quality audits of scanned medical records/documents.

Audits accuracy of electronic and manual entry, and facilitates resolution of any discrepancies. Audits medical records and documents to ensure accuracy of patient identification information and other account information such as date of service, MRN and account number.

May performs medical record deficiency analysis and related medical staff suspension process, if assigned. Assist providers with Meditech usage relative to medical

record deficiency completion and interprets JCAHO standards and medical record staff rules and regulations, as requested. Prepares reports related to medical record deficiency analysis, scanning process and other operational processes, as assigned.

Performs scan preparation/scan/index and quality review of medical records, as assigned.

May perform various administrative duties, including but not limited to, answering/forwarding telephone calls, faxing, typing, sorting mail, and preparation of correspondence. Participates in weekend and Holiday coverage per specific HIM site schedule.

Other duties as assigned.

Education & Qualifications

Requires high school diploma.

* Minimum of one year medical records experience and knowledge of medical terminology required. Ability to use CRT, office equipment and PC.
