RN Case Manager
Looking for a full time RN Cases Manager that is self-motivated. The position is within a Skilled Nursing facility. Candidate should possess passion for geriatric population, most contain a RN license in the state of TN, be able to evaluate and treat independently without direct supervision and possess the desire to perform functional therapeutic activities to assist residents improve in their ADLs.
Job Description:
- Complete a social history and psychosocial assessment for each resident that identifies social, emotional, and psychological needs.
- Ensure and provide support and education to residents/family members/significant others to assist in the understanding of placement and facility issues.
QUALIFICATIONS/REQUIREMENTS:
- RN
- Experience with case management, social work, and/or counseling in a healthcare environment.
